The following commonly used forms will help you through the Financial Aid
process. Please login to
My.Maricopa.edu to view your requested documents, print the requested
forms and submit to the appropriate Financial Aid office.
Each form is presented in PDF format which you can view with the free Adobe Reader.
Verification Forms
Asset
Verification Form - verify student and/or parent asset information.
Dependent Household and Untaxed Income Worksheet - verify dependent student and parent household size and untaxed income.
Dependent
Verification Worksheet - verify dependent student and parent household
size and other income.
FAFSA
Signature Page - paper form that provides student and/or parent
signature to the FAFSA.
Federal Loan
Discharged Due to Disability - determines loan eligibility if a federal
student loan was discharged due to disability.
Homeless
Verification Form - determines living situation and risk of
homelessness.
Independent Household and Untaxed Income Worksheet - verify independent student and parent household size and untaxed income.
Independent
Verification Worksheet - verify independent student and/or spouse
household size and other income.
Selective
Service - verify a student's selective service registration status. Male
students are required to contact Selective Service.
Reviews and Other Documents
Consortium
Agreement - use this form if you will attend two schools during the same
semester. You must make payment arrangements at the participating school or
you risk being dropped for non-payment. Consortium Agreements are subject to
approval and may not be granted if classes are not part of the students
program of study on file, a student has not maintained MCCCD academic
progress or a student is not taking six (6) or more credit hours at GateWay.
Dependency
Review - for students who have special circumstances regarding their
dependency status.
Income Review
- Sometimes families experience special circumstances which merit
recalculating their financial aid eligibility. Reduction of student income
must be at least 20% before the review will be conducted.
Other Forms
FAFSA Parental Data Override Form - documents the lack of parental data on the Federal Application for Federal Student Aid (FAFSA) in order to qualify for Federal Unsubsidized Stafford Loan.
Financial Aid Revision Form - changes an award after an Award Notification has been issued.
Request for Continued Funding - form used to request an evaluation of financial aid
funding based certain circumstances.
Satisfactory Academic Progress Appeal Form - complete and submit to appeal Financial Aid Suspension status.
